Abstract

°á°ú: CT scan ºÎÅÍ 3Â÷¿ø¸ðµ¨ÀÌ ³ª¿À±â±îÁöÀÇ ÃÑ Á¦ÀÛ °úÁ¤Àº ÃÖ¼Ò 5ÀÏÀÌ ¼Ò¿äµÆÀ¸¸ç, CT µ¥ÀÌÅÍ ÈÄó¸®ÀÇ °æ¿ì Æò±Õ 3½Ã°£, ÈÄó¸® ¸ðµ¨¸µÀº 2½Ã°£, ¸ðµ¨ Ãâ·Â¿¡ Æò±Õ 9½Ã°£, ÁöÁö´ë¸¦ Á¦°ÅÇÏ´Â µîÀÇ ¸¶¹«¸® ÀÛ¾÷¿¡ Æò±Õ 1½Ã°£ Á¤µµ°¡ ¼Ò¿äµÆ´Ù. °Ë»çÀÚ Áß 13¸íÀÌ ¼ö¼úÀ» ÁøÇàÇÏ¿´À¸¸ç ¸ðµÎ »ýÁ¸ÇÏ¿´°í, 4¸íÀÇ °æ¿ì´Â ¼ö¼ú ¿¹Á¤ ÁßÀÌ´Ù.

°á·Ð: CT µ¥ÀÌÅÍ ÈÄ󸮴 ¹æ»ç¼±»çÀÇ ¾÷¹« ¿µ¿ªÀ¸·Î ±× ¹æ¹ýÀº ½ÉÀåÀÇ ¸ð¾çÀ» Á÷°üÀûÀ¸·Î º¼ ¼ö ÀÖÀ¸¸ç Á÷Á¢ÀûÀÎ ¼ö¼ú ¹æ¹ýÀ» °èȹÇÒ ¼ö ÀÖ´Â ÀåÁ¡ÀÌ ÀÖ¾î ¼ö¼ú ¹æ¹ýÀÇ °áÁ¤¿¡ Áß¿äÇÑ ¿ªÇÒÀ» ÇÏ¿´´Ù. °á°úÀûÀ¸·Î Á¤È®ÇÑ ¼ö¼úÀÇ ¹æ¹ý°ú ¼ö¼ú½Ã°£À» °í·ÁÇغÃÀ» ¶§ ¼±Ãµ¼º ½ÉÀ庴 ȯÀÚÀÇ CT µ¥ÀÌÅ͸¦ ÀÌ¿ëÇÑ ½ÉÀå 3Â÷¿ø ÇÁ¸°ÆÃÀº °¡Ä¡ ÀÖ°í Áß¿äÇÑ ±â¹ýÀ̶ó »ç·áµÈ´Ù.
Purpose: Most of the congenital heart disease (CHD) patients are newborns or infants. It is very hard to make surgery due to the small size and various form of heart in CHD patients. Therefore, high level of clinical skills are required in the operation procedure. In this reason, a 3D modelling of heart was developed for CHD surgery planning. In addition, the post-processing methods and precautions were studied in 3D modelling process based on CT data.

Material and method: From January to December of 2017, 20 pediatric patients with CHD were selected for surgical planning with 3D modelling. Post-processing of image requires data-based segmentation such as right atrium including superior vena cava (SVC) and inferior vena cava (IVC), right ventricle, pulmonary artery, left atrium including pulmonary vein, left ventricle, and aorta and coronary artery. Also, additional segmentations were required in surgery or treatment.

Result: Overall production of heart takes 5 days from CT scan to printing of 3D model. The average time spent in post-processing of CT data, post modelling, printing, and finishing work are 3 hours, 2 hours, 9 hours, and 1 hour, respectively. 13 patients who underwent surgery were all alive and 4 patients are scheduled for surgery.

Conclusion: Post-processing of CT data is work of radiological technologist. It enables to figure out the shape of heart intuitively and allow efficient planning and decision of the operation methods. Take into account the proper surgery and surgical time, 3D printing of heart using CT data of CHD patients is one of valuable and important technique.
